Diabetic foot syndrome is one of the most serious chronic complications of diabetes. The combination of neuropathy and ischaemia puts patients at risk of ulcers, gangrene, osteomyelitis and phlegmon, with possible amputation. It can also involve destructive disease of the bones and joints of the foot, known as neuropathic Charcot osteoarthropathy. The foundation of treatment is active offloading. Treatment depends on many factors and must always be comprehensive, including diabetes control through pharmacotherapy, appropriate offloading, revascularisation, debridement including biological debridement with larval therapy, and hydro-surgery using the Versajet system as a new debridement method. Other approaches include growth factors and potentially stem cells, skin grafts, Vacuum Assisted Closure, recal cification therapy with bisphosphonates or calcitonin for Charcot osteoarthropathy, and treatment of infection.
