The fourth volume of the Motol Pediatric Seminars editorial series is dedicated to the pediatric skeleton from the perspective of specialists from several medical fields. The pediatric skeleton has been at the center of physicians' attention since ancient times. Serious injuries often affect the skeletal system, and the treatment of fractures has been a concern for doctors since time immemorial. One of the first nationwide preventive programs in pediatrics became the prevention of rickets in infants and toddlers through the administration of cod liver oil and later preparations with a defined content of cholecalciferol, contributing for more than a hundred years to the physiological development of the skeletal system in the entire pediatric population. Recent years have brought new insights into the molecular basis of pediatric skeletal development, its regulation, and the pathological conditions that may accompany the development of the skeletal system. For some children with congenital skeletal diseases, previously treated only palliatively, targeted therapy options are now opening up. Such diseases include X-linked hypophosphatemic rickets, for which a new treatment with burosumab - a monoclonal antibody against FGF23 - is available.
