The publication aims to provide a clear summary of current information on the latest modality of treatment for Crohn's disease and ulcerative colitis, which is continuously evolving. The introductory section discusses the history of biological therapy as well as the individual molecules used not only in gastroenterology, their nomenclature, and mechanisms of action in light of current knowledge of the pathophysiology of idiopathic intestinal inflammation. In the following chapters, the authors address the fundamentals of classification, diagnosis, and therapeutic principles of Crohn's disease and ulcerative colitis. This is followed by a detailed assessment of the role of biological therapy in a comprehensive treatment strategy during remission, relapse, in the therapy of complications, and treatment prophylaxis, including indications, contraindications, drug interactions, and side effects. Compared to previous editions, the section dedicated to surgical intervention has been enriched, including the rarely discussed issue of stomas in idiopathic intestinal inflammation in global literature. The new worldwide epidemiological situation, which has impacted both medicine and the economy and social structures of our society, has inspired the expansion of chapters dedicated to specific situations.
