Description
In the presented publication, the authors aim to objectively and critically evaluate the current perspective on so-called cardiorenal syndromes, demonstrate the pathophysiological connection and mutual influence of the heart and kidneys, contextualize sometimes theoretical tabular classifications into clinical practice, and define (to the benefit of the patient) the real possibilities for the diagnosis and treatment of these diseases. Not only from a pathophysiological standpoint but also therapeutically, the area of cardiorenal syndromes is very broad, encompassing diuretic treatment, ultrafiltration, peritoneal dialysis, renin-angiotensin-aldosterone system inhibitors, inotropic support, the application of nesiritide, adjustment of renal hemodynamics, treatment of anemia, and a wide range of treatments for kidney diseases themselves. The authors also address prospective directions in therapy, such as vasopressin antagonists, A1-adenosine receptor antagonists, serelaxin, and mechanical cardiac support. Nephrologist MUDr. Štefan Vítko, CSc., and cardiologist Prof. MUDr. Věra Adámková, CSc., are leading figures at the Institute of Clinical and Experimental Medicine in Prague (IKEM).
