The use of assessment tools in the comprehensive care of patients with dementia has several meanings. They assist not only in diagnosing the disease but also in identifying the limitations, risks, needs, and priorities of each individual patient. Assessment tools and techniques (scales, ratings) help identify and categorize at-risk patients, objectively assess their functional status, specific needs, and evaluate the effectiveness of the provided nursing care. Measurement tools fill the gap between data obtained from traditional clinical examinations (medical history, physical examination) and paraclinical examinations. The aim of this monograph is to comprehensively address the issue of measurement tools for selected domains in patients with dementia – nutrition, pain, functional status, risk of pressure ulcers, safety, cognitive functions, delirium, behavioral and mood disorders, and quality of life.
