Knee osteotomy is a surgical method for correcting metaphyseal bone deformities of the distal femur or proximal tibia, or both, with the aim of restoring the physiological course of the lower limb’s mechanical axis. Correcting the axis distributes the load more evenly, improves the biomechanics of the knee joint and slows the progression of osteoarthritis, making osteotomy particularly suitable for younger and active patients. The procedure has a long history and has experienced a revival in recent decades thanks to modern surgical techniques, advanced fixation systems and a better understanding of knee biomechanics. Careful indication, detailed preoperative planning and the choice of an appropriate type of osteotomy are crucial. When performed correctly, osteotomy preserves the natural function of the knee joint, improves quality of life and may delay or completely eliminate the need for total knee replacement.
